About the business
The first condition in this business is not money, it is paper. Section 22 of the Bangladesh Medical and Dental Council Act 2010 forbids allopathic practice without registration, and section 43(3) makes it an offence for an unregistered person to hold himself out as a medical or dental practitioner, punishable by three years' imprisonment or a fine of one lakh taka, or both. Diploma-holding dental technologists running their own chambers and seeing patients is a familiar sight in Bangladesh, and it is not lawful. On 13 June 2016 the High Court had directed that they be registered and allowed to examine patients and prescribe; on 24 June 2026 the Appellate Division scrapped that judgment. They may now work only under the supervision of a registered dental surgeon: no chamber of their own, no examining patients, no prescribing. So there is one lawful way into this trade: pass BDS, register with the BMDC, and renew before the validity date printed on the certificate. Then comes the establishment side. The DGHS citizen's charter states that a private health establishment licence is applied for online and takes a minimum of 30 working days from the date the application is properly completed. It requires an up-to-date trade licence, VAT registration, income tax certificate, a waste management contract, a narcotics licence, environmental clearance, and the BMDC registration of every doctor. From the 2026-27 fiscal year that licence runs two years instead of one, and two years of fee and VAT must be paid together by treasury chalan.

How the work is done
- 1Take the BDS degree and get full registration with the Bangladesh Medical and Dental Council. Practising dentistry without registration is forbidden by law, and there is no route into this business that skips this step.
- 2Renew the registration before the validity date printed on the certificate. Renewal needs the form, the original registration certificate, the BDS certificate, an e-TIN with the latest income tax return acknowledgement, and a copy of the national ID.
- 3Decide the form: a chamber inside someone else's clinic, or a dental clinic of your own. The DGHS licence attaches to the establishment, not to the person.
- 4Take the trade licence, e-TIN and VAT registration. Without those three the DGHS application cannot even be completed.
- 5Sign a medical waste management contract. Blood-soaked cotton, used needles and extracted teeth cannot go into ordinary rubbish, and the contract is required with the licence application.
- 6Get the location clearance or environmental clearance from the Department of Environment; it has been made mandatory for new licences.
- 7If narcotic or controlled drugs will be kept, take a narcotics licence. It is applied for separately after establishment approval.
- 8Apply for the licence on the DGHS online portal and upload every document. The charter puts it at a minimum of 30 working days from the date the application is properly completed.
- 9Pay two years of licence fee and VAT together into the government treasury by chalan. From the 2026-27 fiscal year the licence runs two years instead of one. The amount is whatever the latest gazette says; the charter does not print it, so ask the Hospital and Clinic Section before applying.
- 10Keep every doctor's BMDC registration number on file. It is asked for in the licence application and checked again at inspection.
- 11If you employ a dental technologist, they may work only under a registered dental surgeon's supervision. They cannot see patients, run a chamber of their own or prescribe: the Appellate Division settled that on 24 June 2026.
- 12Set up a sterilisation routine: every instrument through the autoclave after use, and every cycle written in a log. An autoclave pressure steam steriliser was Tk 16,500 in September 2026.

Risks and cautions
- HighCompliance: Practising dentistry without registration is a crime, not a business. Diploma-holding technologists opening their own chambers is common in Bangladesh, and the Appellate Division closed that route completely on 24 June 2026 by scrapping the 2016 High Court judgment. Section 43(3) carries three years' imprisonment or a one lakh taka fine or both. Anyone planning to open a chamber on a diploma is not planning a business, they are planning an offence.
- HighCompliance: The licence paperwork is a stack and one missing item stops all of it: trade licence, VAT registration, income tax certificate, waste management contract, narcotics licence, environmental clearance and every doctor's BMDC registration. Even with everything in order it is a minimum of 30 working days, and the rent and wages run through all of it.
- HighDemand: The price competition comes from chambers that carry none of these costs. An unregistered chamber has no licence, no waste contract and no environmental clearance, so it can undercut. A lawfully run clinic has to stand up under that pressure, and patients generally cannot tell the two apart.
- HighWorking capital: The chair, compressor, x-ray and instruments are all paid for before the first patient arrives, and the licence stack on top of that. Income comes patient by patient, eight to twenty-five a day, so the equipment money takes many months to come back.
- HighOther: Liability for infection and for bad treatment falls directly on the dentist, and the BMDC can take disciplinary action against a registered practitioner. A registered practitioner who helps an unregistered person to practise, or keeps a professional association with one, is himself liable to disciplinary action. Carrying on while the autoclave is out of order is the commonest and costliest mistake here.
